RELEASE AND INDEMNITY

You have enrolled in a course offered by the Eastern Safety Services, a Division of Eastern Ventures Inc. commencing the date of this registration form. As a condition of taking this course, I participated in an introductory session given by a staff member of Eastern Safety Services who advised me that the course involves physically and mentally strenuous activity and that participation in the course may be hazardous to me.

I am fully aware and understand the risk to me in taking this course and I have agreed to participate in the course and I have taken whatever additional steps I deemed necessary to satisfy any concerns which I may have had in connection with my participation in the course. In consideration of the Eastern Ventures Inc. agreeing to enrol me in the course, I release the Eastern Ventures Inc., its instructors, officers, servants, employees and agents from any claim which I or my personal representatives may have against Eastern Ventures Inc., its instructors, officers, servants, employees, and agents for damages in respect of personal injury, death, sickness, accident, loss of wages, income, benefits, employment opportunities or other harm suffered in connection with or directly or indirectly arising out of my being enrolled and participating in the course except where such damages are attributable to the negligence of the Eastern Ventures Inc., its instructors, officers, servants, employees and agents, and I agree to indemnify the

Eastern Ventures Inc., its instructors, officers, servants, employees and agents in respect to any such claim which may be brought against any one or more of them in connection therewith by any person, including my personal representatives and any person claiming on my behalf and any employer claiming for loss or lessening of my services.
